Rochester Senior Support Services offers many solutions. Out of town family- We know it is difficult to care from a distance. You have your life and responsibilities and that cannot change. You love and respect your family member and want to help but are frustrated by the distance and lack of communication. We understand, we have helped many clients in the same situation. Allow us to be there for you and your loved one. Bill Minding - We come in regularly and write out checks and have your loved one sign the check. In some cases we may be able to even help setup automated payments using the technology available today. You will have the peace of mind of knowing that the lights stay on and the phone is working. We can also help register your loved one for the 'Do Not Call' registry to help protect them from fraudulent/annoying telemarketers. Health Advocate - We can be the 'translator' between all the doctors and nurses and your loved one. We would coordinate and accompany your family member to all appointments and be in the room during the exam. We then would be able to inform the family of exactly what is going on...and how things are looking. You are ALWAYS in the loop and not just told that 'everything is fine'. Assessment and Care Planning - An assessment is comprised of an examination of the older person's phsyical, mental and psychological health. It also tests their ability to complete activities of daily living and social support. That information is reviewed and anticipation of current and future needs are identified. A comprehensive care plan is then made to 'solve' or intervene any problems identified. Services then may be put in place depending on the wishes/needs of the family/loved ones. On Going Care Management - After all of the planning has been done and implemented, Rochester Senior Support Services can then be your 'eyes and ears' to monitor the situation and anticipate any 'speed bumps'. Sometimes this can mean visiting periodically with your loved one to check in on things. It can also mean 'crisis support' if you are out of town or otherwise unavailable to manage a crisis situation such as a hospital visit or other major issue. You can have the peace of mind knowing that they are not alone and are comfortable. In addition you would have a set of eyes and ears to be able to relay all information to you in a timely and compassionate manner. Change of Residence - Whether moving from a large house to an apartment or from an assisted living arrangement to a nursing home (or anything in between) moving can be stressful for your loved one and you. We have the connections and knowledge to make this stressful time smoother and easier. We will help you select an independent, assisted or skilled nursing facility. We then would coordinate the move, follow up with your loved one and help ease the transition for them any way that we can. Moving is one of the biggest stresses there is but with our help we hope to make it easier and to take the load off you. Consultation - Sometimes families just need a direction to go in. We can give our recommendations or referrals. Often we find that there are many loving and caring family members that can assist in carrying out and monitoring a plan. They just need to know what the plan is. We want to help with that. Benefit Applications - In many situations your loved one may be eligible for benefits such as: Veteran's Benefits, Medicaid, EPIC and much more. In most circumstances we can help fill out the application and handle follow-up. For Medicaid we work with many wonderful elder law attorneys and know how to help your loved one get the most they are entitled to. Public Speaking/Seminars - Need a speaker for your next event? She has done many presentations on various topics related to aging and care on all levels. She would love to bring her knowledge and expertise to your forum. |
